To sign-up to play in the tournament, just print out and complete the registration form and mail it, along with your check, to:

Tournament Basics

• Date: Monday, September 13, 2004 (Raindate: September 14)
• Time: 8:30am
• Place: Concord Country Club
• Cost: $135 per person (includes green fees, cart, lunch and prizes)
• Teams: Play will be in teams of 4. Get a team together, or sign-up alone, and we will match you with a team.

The Day's Events

• 8:00am: Registration begins; Continental Breakfast
• 8:30am: Shot Gun starts
• After golf: Lunch and Awards Ceremony
